This drug was compounded for you by O’Brien Pharmacy according to your provider’s prescription. CAUTION Federal law prohibits the transfer of this drug to any person other than the patient for whom it was prescribed. This information is provided in addition to the written information and pharmacist consultation you received. Use 
This is a compounded medication that is specially prepared by a pharmacist to meet an individual patient’s needs according to your provider’s prescription.  

Side Effects 
Side effects will depend on the active ingredients and route of administration. Contact the pharmacy for information regarding possible side effects. Seek immediate medical attention if you are experiencing a medical emergency.  

Precautions 
Do not use this medication if you are allergic to any ingredients. Most ingredients can be found on the label, but always inform the pharmacist of your allergies and sensitivities. You should tell your provider and pharmacist if you are pregnant, planning pregnancy, or breastfeeding, or have chronic medical conditions. You should provide your provider and pharmacist with a current list of prescription, over-the-counter, and herbal products. 

Directions for Use 
Use this medication exactly as prescribed by following the instructions on your prescription label and counseling provided by your pharmacist carefully. If you are unsure how to use this medication, contact your pharmacist for guidance. Typically, if you miss a dose, you will take it when you remember unless it’s closer to the next dose. Doubling up is typically not advised. Contact your pharmacist if you don’t know what to do after a missed dose. 

Storage 
Compounded medications tend to have shorter shelf lives than other drugs and often need cold storage. Storage requirements are listed on the label. Protect from light. Take note of the beyond use date and discard after labeled date. Caution Packaging may not be childproof, keep out of reach of children and pets. Do not flush unused medications or pour down a sink or drain. 

You have the right to know about the proper use of your medication and its effects. Patients in California will receive a return call on Saturdays for voicemails left by noon Central Time (10 a.m. Pacific Time).
